Program for providing medicine for the chronic diseases

 

I.Who is involved in the program:

Socially  Vulnerable persons of any age whose family rating score is defined by 100,000 and have  the following chronic diseases:

  • Cardiovascular diseases
  • Chronic diseases of lung;
  • Thyroid gland diseases.
  • Diabetes Type 2  (Non  insulin-dependent)

 

II.What procedures are required to get medicine?

  • The beneficiary goes to the family (rural) doctor or physician specialist who writes a form of confirmation of chronic disease. The doctor prescribes the diagnosis of the disease and the daily dose of the medicine.
  • The doctor writes a prescription indicating an international, not patented drug name.
  • Beneficiary with the Form N100, prescription and the ID Card, goes to the nearest department of the Social Service Agency, where he/she is registered for the program.
  • After registration in the Social Service Agency, the beneficiary addresses the PSP’s relevant Pharmacy.
  • The beneficiary can at once take medicine that will be enough for three months.
  • At the pharmacy beneficiary will present a prescription issued by the doctor and identity document
  • The beneficiary’s trusted person will additionally present his / her identity card
  • In the case of taking each medicine, a person pays 10% of the medicines value, which does not exceed 1 lari, but is not less than 5 tetri.

 

III.Additional Information:

  • In case of changes in the deduction or dosage, social service agency will submit the beneficiary an updated form 100 and the person will be re-registered in the program

•If during medicine supply, the beneficiary family’s rating points will change in the database of vulnerable people and exceed 100,000, the person will not be able to receive medicine.
